Does anyone know what impact all these extra horses will have on the stock transmission and/or read end.

The trans is a truck tranny it can handle it no issues, the differential is junk to begin with so who knows. I think Nubbs is racing a GXP with the stock differential and he is hitting a lot more HP and torque than we are. They use this same differential in one of the Cadillac's that has a lot more HP than we do so I think we are OK.

If you take all the "slack" out of the rear gears before you do any burnouts , or starts at the dragstrip , you'll be doing as much as you can to prolong the life of the rear gears. Me, I prefer rolling starts..........

Has anyone switched to a Cad CTS-V differential? Is that possible easily?

No need for me since I am stock. Too bad the Westers tune would void my warranty.

Anyway, my guess is the V differential would be an easy swap. Something to consider for those into heavy mods or drag racing.
